Register for TJAY Soccer for SPRING 2012
(TJAY=Thomas Johnson Area Youth)

We have three types of soccer registrations: In-House, Double-Nickel, and County. The In-House league (for children ages 4-6 on July 31) is played entirely on our local field, with emphasis on development, sportsmanship and fun. The Frederick County Double Nickel League for children aged 6-7, is a community-based league focusing on building confidence and player development. The County teams, for children age 8-14 (8-18 in the spring), play in the Frederick County Youth Soccer League (FCYSL), and compete at all levels with teams all over the County.

SPRING 2012 Registration Instructions

There are 3 ways to register for TJAY Soccer:

1. Register on-line and pay on-line.
2. Register on-line and then mail in your payment of check, cash, or money order.
3. Download the registration form, print, and mail it with your payment (links to the In-House and County registration form PDFs appear below).
4. Download the Financial Aid Form if you require payment assistance.

**Waiver Forms ** If you are registering on-line, you will need to check the waiver box in order for your registration to be complete. If you are mailing in a paper form, you will need to be sure that your waiver is attached. The waiver is the 3rd page of the paper registration pdf file.

**Payment** If your payment has not been received by the first game, your child will not be able to participate until payment is received. If you are having financial difficulties, please complete the financial aid form and mail this in with your registration.

**Birth Certificates** Also, to ensure that players are playing in their correct age brackets, FCYSL requires that we keep a copy of your child's birth certificate on file. If you have not turned in a copy of your child's birth certificate in the past, you will need to send this in at the same time as your waiver.

**Uniforms** If you need a uniform, be sure to make a note of it on your registration form! (This only applies to County teams). Also, remember to indicate your size.

Official TJAY Uniform

• All participants in the County league must have an official TJAY uniform. If your child does not have a uniform to wear for the County league season, add $30 to the registration fee. If you need to order a uniform, please indicate the size on the enrollment form. TJAY soccer will not sell uniforms piece-meal.
